Salary: $45.68/hr. – $60.36/hr. DOE

Some patients need more than an appointment. They need a clinician who can understand the behavioral, medical, and social factors affecting their health and help them remain connected to the care they need.

Jamaica Hospital Medical Center is seeking a Psychotherapist (LMSW) to join our Mental Health Clinic as part of our Center of Excellence (COE) Program.

The program serves patients at increased risk for emergency department visits and hospital admissions, including individuals whose psychiatric and physical health conditions may be complicated by Health-Related Social Needs (HRSNs).

What You’ll Do

• Conduct initial and ongoing diagnostic assessments
• Develop and implement individualized treatment plans
• Provide individual, family, and group psychotherapy
• Coordinate care with psychiatric providers and the multidisciplinary healthcare team
• Address behavioral health and social needs that may create barriers to care
• Participate in discharge planning, follow-up, and continuity-of-care efforts
• Maintain accurate and timely clinical documentation
