Community Outreach Coordinator – Health Equity
Location: Jamaica Hospital Medical Center – Queens, NY
Department: Health Equity Initiatives
Type: Full-Time | Administrative/Clerical
Hours: 8:00 a.m. – 4:30 p.m. or 8:30 a.m. – 5:00 p.m.
Salary: $90,000 – $100,000

Jamaica Hospital Medical Center is seeking a dynamic and dedicated Community Outreach Coordinator to join our Health Equity Initiatives team. In this role, you will build bridges between the hospital and the community, fostering partnerships that enhance patient- and family-centered care while advancing health equity.

What You’ll Do:

  • Lead the development and management of a system-wide Patient and Family Advisory Committee (PFAC), recruiting members and facilitating regular meetings.
  • Build and maintain strategic relationships with community-based, faith-based, and non-governmental organizations, local businesses, and other stakeholders to reduce healthcare disparities.
  • Deliver educational programs and presentations to patients, families, and community members.
  • Represent JHMC at health fairs, community events, and outreach initiatives.
  • Support fundraising and development efforts, including donor management, grant proposal support, and campaign strategies.
  • Perform additional duties as assigned to support departmental goals.

Requirements
  • Bachelor’s degree in Public Administration, Public Health, Social Work, or a related field.
  • Minimum 3 years’ experience working in an urban community setting with expertise in community health promotion, outreach, or capacity building.
  • Respectful, empathetic, and culturally sensitive approach to diverse populations.
  • Ability to work independently and collaboratively in a team environment.

 

Preferred Qualifications:

  • Master’s degree in public administration, Public Health, Social Work, or related field.
  • Fluency in Spanish and/or Haitian Creole in addition to English.
