COMMUNITY HEALTH WORKER –  PROGRAM FOR DIABETES IN PREGNANCY

Jamaica Hospital Medical Center’s Program for Diabetes In Pregnancy, offers a variety of comprehensive care and services for pregnant women with diabetes. The program includes in-home assessments, medication management, nutritional education and support services to address needed housing stability, food security, and access to healthcare.

We are currently seeking compassionate and dedicated Community Health Workers to join our team. This is a full-time opportunity for individuals who are passionate about helping pregnant women with diabetes receive the support and care they need for a healthy pregnancy.

RESPONSIBILITIES

  • Selected candidates in this role   will be responsible for performing in-home accommodation assessments to document home conditions and any environmental factors that may affect the patient’s health or diabetes management.
  • Observing and recording specific concerns or anxieties the patient may have regarding their pregnancy and the impact on their health.
  • Documenting patients Diabetes Care Compliance their ability to adhere to diabetes care regimen, including medication adherence and blood glucose monitoring.
  • Assessing and recording patient’s dietary habits and any barriers to maintaining a diabetes-friendly diet.
  • Tracking and documenting glucose levels to ensure compliance with diabetes care protocols.
  • Monitoring and recording weight management as part of overall pregnancy and  diabetes care.
  • Evaluating patient’s physical activity levels and providing encouragement or resources to maintain appropriate exercise during pregnancy.
  • Assessing mental health status, including stress, anxiety, or depression, and referrals to appropriate mental health support if needed.

Requirements
  • Community Health Worker Certification
  • Must be flexible and able to adapt to varying work schedules, including evenings and weekends.
  • Must be willing to travel within the community as required for outreach and support initiatives.
  • Prior experience working with medically underserved populations is essential, as this role requires a deep understanding of the unique challenges these communities face.
  • Strong interpersonal skills to effectively engage with clients and collaborate with healthcare teams. Good command of the English language.
  • Ability to work sensitively and effectively with diverse populations.
  • Projects a professional demeanor.
  • Associates degree recommended; Bachelor’s degree preferred.

SALARY:  $65,000.  Plus Full Benefits & PTO
