MediSys Family Care Center in St. Albans provides a comprehensive range of general and specialized medical services, including Family Medicine, Pediatrics, Neurology, Rheumatology, Prenatal Care, and more. We are currently seeking a compassionate and dedicated Social Worker (LMSW/LCSW) to support our patients and enhance their overall well-being.

Key Responsibilities:

• Assess patients’ social, emotional, and financial needs to identify barriers to care.
• Provide therapy and short-term counseling to patients and families coping with illness and stress.
• Advocate for patients by connecting them to essential healthcare services and community resources.
• Collaborate with the medical team to develop personalized care plans.
• Assist with discharge planning, ensuring proper follow-up care and home health arrangements.
• Offer crisis intervention and emotional support as needed.
• Educate patients and families about medical conditions, treatment options, and coping strategies.
