Temporary Medical Social Worker – NYS LMSW Required

This is a temporary full-time Social Worker position. The assignment is for approximately 9 months in length. Please apply if you are interested in this opportunity. You must be a NYS Licensure (SW) to be considered.

Responsibilities for Medical Social Worker:

• Collaborate with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs
• Act as an advocate for patients to resolve crises and provide appropriate interventions
• Investigate suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
• Complete a biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
• Refer patients to community or social service programs to meet their complex needs
