Case Manager – Case Management

Supplying Managed Care insurers with documentation supporting acute inpatient hospitalization for assigned patients on their unit; participates in denial management for assigned patients on their unit whose inpatient services have been denied; evaluates the need for home care services for all assigned patients on their unit to ensure a safe and timely discharge; identifies potential barriers and collaborates with the health care team to resolve them in order to ensure a safe and timely discharge.
