The Department of Ambulatory Care at Jamaica Hospital Medical Center is searching for Licensed Practical Nurses for the opening of our new clinic. Our goal is to cultivate a strong relationship between the patient and their PCP to ensure continuity of care, collaboration, and best outcomes. We see patients of all ages from newborn to geriatrics.
Our LPNs obtain pertinent vital signs, take medical histories, administer vaccines, perform point of care testing, and keep the patient flow moving. They also respond to patient phone calls, provide follow up, and review prescription refill requests with providers. They play a critical role in ensuring that our patients feel informed, satisfied, and in control of their own health.
CORE RESPONSIBILITIES:
• Collects patient data to be used by the licensed health care provider in the comprehensive care of the patient. Obtains vital signs, allergies, current medications, history, and chief complaint, and documents them appropriately in the electronic medical record.
• Responsible to identify problems within the practice and communicates them to their manager. Assists in maintaining a clean and organized department to ensure a safe patient care environment. Supports all initiatives of the health care team and promotes excellence in customer service.
• Prepares charts for the providers in order to assure that the needed information is present on each chart before the patient is seen in the office.

Requirements
EDUCATION: • Graduate of an accredited School of Practical Nursing • New York State licensure. • Current BLS certificate from American Heart Association. • EXPERIENCE AND REQUIRED SKILLS: • One year experience required. • Outpatient ambulatory care clinic experience preferred but not required. • Good communication skills, both verbal and written.
