Make a Difference When It Matters Most
Join our team as a CTI Social Worker and provide community-based behavioral health support services throughout Queens.
In this role, you will work with individuals experiencing serious mental illness and behavioral health challenges as they transition from hospitals, shelters, treatment programs, and other institutional settings into the community. You will conduct comprehensive assessments, provide supportive counseling and crisis intervention, develop person-centered service plans, and connect individuals and families with critical community resources and ongoing treatment services.
Key Responsibilities
- Conduct psychosocial assessments and ongoing client evaluations.
- Develop individualized transition and service plans.
- Provide crisis intervention, supportive counseling, and safety planning.
- Connect clients to mental health, medical, housing, and community-based resources.
- Coordinate care with hospitals, outpatient providers, shelters, ACT teams, and other community agencies.
- Support clients during critical transition periods to reduce hospitalization and promote long-term community stability.
- Advocate for clients and families to ensure continuity of care and access to services.
- Maintain timely and accurate clinical documentation in accordance with program requirements.
