Jamaica Hospital Medical Center is seeking compassionate, dedicated Licensed Master Social Workers (LMSW) to join our inpatient team.

As one of New York City’s busiest hospitals, Jamaica Hospital Medical Center provides social workers with the opportunity to make a meaningful impact on the lives of patients and families facing complex medical and psychosocial challenges. Working collaboratively with physicians, nurses, case managers, and interdisciplinary teams, you will play a critical role in patient advocacy, crisis intervention, discharge planning, and care coordination.

Whether you are an experienced healthcare social worker or a recent graduate looking to begin your career in medical social work, we encourage you to apply.

Responsibilities

  • Conduct psychosocial assessments and evaluate patient needs
  • Develop and coordinate safe, effective discharge plans
  • Provide crisis intervention, supportive counseling, and patient advocacy
  • Collaborate with interdisciplinary healthcare teams to address psychosocial barriers to care
  • Connect patients and families with community resources, support services, and post-acute care programs
  • Facilitate referrals for rehabilitation, home care, behavioral health, housing, and other community-based services
  • Maintain timely and accurate documentation in the electronic medical record

Requirements
  • Master’s Degree in Social Work (MSW) required
  • Current New York State Licensed Master Social Worker (LMSW) required
  • Hospital, medical, discharge planning, or healthcare experience preferred
  • New graduates are welcome to apply

  • Ability to work in a fast-paced acute care environment
  • Rotating weekend and holiday coverage required
