NURSE LIAISON – CARE TRANSITION

The Nurse Liaison role at Jamaica Hospital Medical Center offers exceptional  opportunities for experienced RNs looking to specialize in telephonic case management, care coordination, and transitional care. The position emphasizes post-hospitalization follow-up, chronic disease management, and collaboration with providers to optimize patient outcomes in underserved communities.

Key Responsibilities:

  • Provide telephonic case management in specialized program areas such as Transition of Care, Complex Case Management, Co-Managed Care, or Home-Based Services.
  • Engage with PCPs to ensure post-hospitalization follow-up and continuity of care.
  • Conduct medical record reviews using EMR data to assess patient health status.
  • Develop and manage care plans within the nursing scope of practice.
  • Educate patients on disease management, prevention, and healthy lifestyle habits.
  • Collaborate with patients, caregivers, physicians, and community specialists to achieve individualized care goals.

Requirements

Ideal Candidate:
✔ BSN degree & NYS RN License (Required)
✔ 1-2 years of inpatient nursing experience
✔ Complex Case Management experience preferred
✔ Strong EMR skills & care plan development expertise

Behavioral Health NL – ((1 Opening)

At least one year experience in a behavioral health program/service (i.e.: Psych ED/inpatient psychiatric unit; chemical dependency unit/substance abuse treatment program)
Ability to travel within Queens and Brooklyn for home/community visits when indicated.   Must have own car. Travel reimbursement provided.

Home visiting NL – (1 Opening)

Ability to travel within Queens and Brooklyn for home/community visits required.   Must have own car. Travel reimbursement provided.
Salary: $53.18/hr.
