Flushing Hospital Medical Center, a 293-bed, not-for-profit teaching hospital and member of the MediSys Health Network, is seeking a Psychotherapist to join our Mental Health Clinic as part of our Center of Excellence (COE) Program.

This grant-funded program focuses on providing enhanced behavioral health services for patients who are at high risk for emergency department visits and hospital admissions. The program supports individuals with psychiatric and physical health conditions that may be impacted by Health-Related Social Needs (HRSNs), with the goal of improving patient outcomes through comprehensive care coordination and therapy services.

Position Overview
The Psychotherapist will provide diagnostic evaluation, psychotherapy services, and care coordination to support patients enrolled in the COE program. This role works closely with the program’s Psychiatric Nurse Practitioner and multidisciplinary care team to ensure continuity of care and improved patient outcomes.

  • Key Responsibilities
  • Conduct initial and annual diagnostic assessments
  • Develop and implement individualized treatment plans
  • Provide individual, family, and group psychotherapy
  • Coordinate care with psychiatric providers and other healthcare professionals
  • Assist patients with behavioral health and social care needs
  • Participate in discharge planning and follow-up care coordination
  • Maintain accurate and timely clinical documentation

Requirements
  • Licensed in Mental Health Counseling, or related behavioral health field
  • New York State licensure required (LMH)
  • Experience in outpatient mental health or behavioral health settings preferred
  • Strong clinical assessment, documentation, and care coordination skills
