Social Worker – Oncology

Jamaica Hospital Medical Center currently seeks a Licensed Social Worker for an immediate full-time opportunity within our Oncology outpatient ambulatory clinic. As a member of our outpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understand the impact of their diagnosis, and coordinate complex hospital discharges. Additional duties will include:

Collaborating with other professionals to evaluate patients’ physical and/or mental condition and to assess their discharge needs
Acting as an advocate for patients to resolve crises and provide appropriate interventions
Investigating suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
Completing biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
Referring patients to community or social service programs to meet their complex needs
