The Department of Case Management at Jamaica Hospital Medical Center provides chronic care management programs for patients. Our care management teams and programs support shared decision-making with the patients and families we serve.  We are currently seeking Case Management Assistants for immediate full-time opportunities.    

RESPONSIBILITIES:

Selected candidates will assist interdisciplinary teams in facilitating patient individualized care management and discharge planning services.

DUTIES WILL INCLUDE:

  • Assisting patient/patient family members with information and referrals for Nursing Homes,  (Dialysis Centers and other community resources).
  • Utilizing ePACES to request and receive HIPPA-compliant Claim, Eligibility, Claim Inquiry and Prior Approval/DVS transactions.
  • Utilizing ECIN AND ECIN software systems to input, update and transmit patient information.  
  • Acting as a liaison between Nursing Homes, Case Managers and Social Work Staff.
  • Documenting and updating patient charts in EPIC.
  • Coordinating and managing patient transportation referrals, DME acquisitions and residential facility placements.  
  • Processing Managed Care denials and approval requests.
  • Assisting in quarterly report compilation.
  • Performing unrelated duties in times of emergency
  • Performing other duties as assigned or when necessary.
Requirements
  • High School education required.  Bachelor’s Degree required
  • Case Management Office experience preferred.
