Shift:

  • Five days a week (Monday – Friday).
  • Shift schedule is 8:30 am – 5:00 pm, weekends and Holidays.
  • Holiday coverage is required on a rotating basis.

Responsibilities for Medical Social Worker:

  • Collaborate with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs
  • Act as an advocate for patients to resolve crises and provide appropriate interventions
  • Investigate suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
  • Complete a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan.
  • Refer patients to community or social service programs to meet their complex needs
Requirements
  • Master’s Degree in Social Work

  • New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
  • Hospital/Medical experience preferred but new grads are welcome
  • Knowledge of medical terminology
  • Understanding the roles of the healthcare team
  • Exceptional interpersonal skills
  • Deep understanding of the psychosocial aspects of illness and health
  • Knowledge of community-based organizations and resources for discharge planning
  • Superb written and oral communication skills
  • Strong crisis intervention and short-term counseling skills
  • Excellent time management skills and ability to work in a fast-paced work environment
