Responsibilities for Medical Social Worker:
– Collaborate with other professionals to evaluate patients’ physical and/or mental condition and to assess their discharge needs.
– Act as an advocate for patients to resolve crises and provide appropriate interventions
Investigate suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary.
– Complete a biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
– Refer patients to community or social service programs to meet their complex needs

Requirements
If you’re ready to join the team, requirements are listed below: - Master’s Degree in Social Work - New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) - Hospital/Medical experience preferred but new grads are welcome - Knowledge of medical terminology - Understanding the roles of the healthcare team - Exceptional interpersonal skills - Deep understanding of the psychosocial aspects of illness and health - Knowledge of community-based organizations and resources for discharge planning - Superb written and oral communication skills - Strong crisis intervention and short-term counseling skills - Excellent time management skills and ability to work in a fast-paced work environment
