SOCIAL WORKER (LMSW / LCSW)

The Department of Social Work Services at Jamaica Hospital currently seeks Licensed Medical Social Workers for immediate full-time opportunities within our inpatient Medical Surgical and Float units.  As a member of our inpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understand the impact of their diagnosis, and coordinate complex hospital discharges. Additional duties will include:

• Collaborating with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs
• Acting as an advocate for patients to resolve crises and provide appropriate interventions
• Investigating suspected child abuse or neglect cases and initiating referrals to State Central Registry as necessary.
• Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan.
