SENIOR CARE COORDINATOR

Jamica Hospital Medical Center is seeking a passionate Senior Care Coordinator for an immediate full-time opportunity within our Care Management department. As a member of our team, you will utilize your expertise and knowledge to provide personalized support to patients in and around the community.

Required Skills/Duties:

• Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning in order to establish a person-centered care plan.
• Collaborating with medical and behavioral health professionals towards achievement of health and wellness related goals.
• Acting as an advocate for patients to resolve crises and provide appropriate interventions and short-term coaching/supportive counseling to patients and families.
• Able to investigate financial entitlements, social and community resources.
• Referring patients to community or social service programs to meet their complex needs.
