Jamaica Hospital Medical Center a member of the Medisys Health Network is a 404 bed fully accredited community-based teaching hospital and Level I Trauma and Stroke Center. Our Care Management Department offers a wide range of services including Care Transitions, EMPOWER, Co-Managed Care, and Complex Case Management. These free programs are designed to assist people who require frequent medical attention. Our goal is to improve the health of our community by working closely with our patients and their families as well as their primary care physicians and other health providers. We are seeking an F/T experienced Nurse Liaison Supervisor to oversee our Care Management Team.
SUMMARY
Under the direction of the Director of Care Management/Vice President of Care Management Services, the selected candidate will coordinate readmission reduction initiatives for Care Management Department.
DUTIES
- Responsible for coordinating and managing day to day workflow, administrative and operational functions for the Care Management team
- Monitoring intake referral activities, case records, and EMR documentation.
- Maintaining documentation/statistics to monitor productivity and adherence to departmental and program objectives,
- Monitors and reports attendance and adherence to all departmental and organizational policies and procedures
- Provides coaching, counseling, and administration of corrective action.
- Providing oversight, education, and guidance to Care Management Team to ensure excellence in care, productivity, and operations.
- Directing and instructing non-clinical support staff with regard to the management of the patient, appropriate documentation, and health coaching/education.
- Gathers and enters relevant data in appropriate spreadsheets for tracking and monitoring of team functions and outcomes
- Consistently reviewing and evaluating operational workflow data and reports to facilitate and improve program key performance indicators, outcomes, efficiency, and process.
- Participate in interdisciplinary patient-centered care plan meetings
