Nurse Liaison/Community Nurse

The Nurse Liaison coordinates the placement of all admissions and transfers in accordance with standards and guidelines. He or she will provide clinical follow-up to all discharge patients, plan frequency of follow-up clinical assessments as well as educate patients on diagnosis, treatment/ medications and their side effects. The nurse will be required to accurately document all contacts with patients in EPIC. This position is important in fostering a collaborative relationship with the patients and the care team.
