The Division of Infectious Disease at Jamaica Hospital Medical Center provides treatment to patients diagnosed with infectious disorders and we currently have a career opportunity for a Social Worker to join the team.  In this role, the Social Worker will provide support in linkage to care, and treatment and retention of patients at risk of HIV/AIDS.  Additional responsibilities include:

Responsibilities of Licensed Social Worker:

  • Provide case management and appropriate social support services for active HIV positive patients.
  • Weekly review all outpatient and inpatient HIV patients to link and enroll them to care services with an HIV service provider, and submit data to the HIV Data Coordinator regarding status with patient enrollment and care provided.
  • Refer patients to our linked Community Based Organizations for services not provided at Jamaica Hospital.
  • Attend and participate in NYSDOH AIDS Institute meetings regarding the hospital’s HIV Performance Improvement Activities 
  • Attend at least 24 hours of continuing education courses on HIV/AIDS/STD/HEP, A B C at NYSDOH/AIDS Institute approved training agencies.
  • Perform other related duties and assignments as necessary. 

 

Requirements
  • NYS Social Work licensure required
  • HIV Advance Case Management training / certification  preferred 
  • Knowledge of medical terminology
  • Exceptional interpersonal skills
  • Deep understanding of the psychosocial aspects of illness and health
  •  Knowledge of community-based organizations and resources for discharge planning
  • Superb written and oral communication skills
  • Strong crisis intervention and short-term counseling skills
  • Excellent time management skills and ability to work in a fast-paced work environment
