  • The LCSW (we are accepting both LMSW's and LCSW's) will provide professional casework services to patients and family members with social and or personal problems related to their illness, hospitalization, and readjustment to the community.
  • Provide case management, crisis intervention, and client advocacy.
  • Collaborate with other professionals to evaluate patients' mental condition and assess their discharge needs
  • Counsel clients to assist them in dealing with mental illness.
  • Act as an advocate for patients to resolve crises and provide appropriate intervention
  • Refer patients to a community or social service program to meet their complex needs
  • Complete a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan.
Requirements
  • LMSW or LCSW required. (Provisional License holders may also be considered)
  • Hospital/Medical experience is preferred but new grads are welcome
  • Knowledge of community-based organizations and resources for discharge planning
  • Ability to multi-task and prioritize in a fast-paced, high-volume setting.
  • Excellent communication, organizational, and time management skills. PC Proficiency is required. Superb written and oral communication skills
  • Strong crisis intervention and short-term counseling skills
