PATIENT INFORMATION REPRESENTATIVE -HOSPICE

PLEASE NOTE:   OUR HOSPICE UNIT IS  IN NEED OF A PATIENT  INFORMATION REPRESENTATIVE.  THIS POSITION INVOLVES WORKING IN OUR HOSPICE UNIT WITH TERMINALLY ILL  PATIENTS AND THEIR FAMILIES.  THE ROLE WILL INVLOVE REGULARLY DEALING WITH DEATH AND GRIEF.  IF THIS IS NOT THE TYPE OF  SETTING YOU WOULD BE COMFORTABLE WOKING IN DAY AFTER DAY,

FULL TIME MONDAY – FRIDAY  10am – 6:30pm

Jamaica Hospital Medical Center’s Ferrara Family Center for Hospice Care is a 10-bed unit. Our Hospice Center provides comfort care for those with life-limiting illnesses; and our goal is to enhance each person’s quality of life medically, spiritually and emotionally.  We are currently seeking a full-time Patient Information Representative for immediate opportunity in our in our Hospice Unit.

DUTIES

• Utilizing EPIC software to input, update and retrieve patient data.
• Assisting patients in obtaining services, understanding policies and making health care decisions.
• Assembling charts for patient admissions.
• Coordinating communication between patients, family members, medical staff, administrative staff, or regulatory agencies.
• Interviewing patients or their representatives to identify problems relating to care.
• Maintaining knowledge of community services and resources available to patients.
• Referring patients to appropriate health care services or resources.
• Investigating and directing patient inquiries or complaints to appropriate medical staff members.
