Flushing Hospital in Queens is seeking a dedicated and compassionate Social Worker with an LMSW, LCSW, or LMHC certification to join our Addiction Services team. The ideal candidate will have a minimum of three years of relevant experience in providing support and counseling to individuals struggling with substance use disorders. This role involves working closely with patients, families, and healthcare professionals to develop and implement effective treatment plans that promote recovery and well-being.

Key Responsibilities:

Assessment and Evaluation:

Conduct comprehensive biopsychosocial assessments to determine patients’ needs, strengths, and treatment goals.
Utilize standardized assessment tools to evaluate the severity of substance use disorders and related psychosocial issues.
Develop individualized treatment plans in collaboration with patients, families, and multidisciplinary teams.
Provide evidence-based therapeutic interventions, including individual, group, and family counseling.
Facilitate psychoeducation groups to enhance patients’ understanding of addiction and recovery processes.
Coordinate care with other healthcare providers, community resources, and support services.
Monitor patients’ progress and adjust treatment plans as necessary.
Advocate for patients to ensure they receive the appropriate services and support.
Provide immediate support and intervention during crises, including risk assessment and safety planning.
Collaborate with emergency services and psychiatric teams as needed.
Maintain accurate and up-to-date clinical records in compliance with hospital policies and regulatory standards.
Prepare reports and summaries for interdisciplinary team meetings and case conferences.
Participate in ongoing training and professional development activities to stay current with best practices in addiction treatment.
Supervise and mentor junior staff and interns, if applicable.

Requirements
Master's Degree in Social Work (MSW), Counseling, or a related field. Current and valid LMSW, LCSW, or LMHC licensure in New York State. Minimum of three years of relevant experience in addiction services or substance use disorder treatment. Strong understanding of addiction treatment modalities, including Motivational Interviewing, Cognitive Behavioral Therapy, and Trauma-Informed Care. Excellent communication and interpersonal skills. Ability to work effectively in a multidisciplinary team environment. Proficiency in electronic health record (EHR) systems.
