Flushing Hospital Medical Center has a full-time career opportunity for a Clinical Pharmacy Supervisor /Internal Medicine.

This is a hybrid position entailing responsibilities both on the inpatient and outpatient care settings.

The Clinical Pharmacy Coordinator will round with multi-disciplinary teams for general medicine/surgical patients and provide pharmacotherapeutic management to patients. In addition, this individual will attend a general medicine ambulatory care center onsite at the hospital to set up and run a disease state management clinic.

Inpatient Internal Medicine Responsibilities

  • Practice with other members of the patient care team and focus on the needs of patients.
  • Regularly make rounds as a member of the multidisciplinary patient care team to optimize pharmacotherapeutic managements of patients.
  • · Prospectively evaluate and document all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitors the patients’ pharmacotherapeutic regimen for effectiveness and ADEs; and intervenes as needed.
  • Identify ADEs and assists in their management and prevention, and develops process improvements to reduce drug errors and preventable ADEs.
  • Provide drug information to the patient care team.
  • Provide drug therapy related education to healthcare team.
  • Provide didactic lectures to health professional residents and students in pharmacology and therapeutics, where applicable.
  • Perform drug utilization review.

Ambulatory Care Responsibilities

  • Provide direct patient care and medication management for ambulatory patients, long-term relationships, coordination of care, patient advocacy, wellness and health promotion, triage and referral, and patient education and self-management.
  • Assure provision of safe, efficacious and cost-effective drug therapy for all patients.
  • Work in a multidisciplinary team and establish a collaborative drug therapy management practice model.
  • Provide face-to-face and one-on-one phone based disease management with patients.
  • Utilize integrated and routine communication with all health care professionals and the patients concerning medical care and quality of life with the emphasis placed on the monitoring and documentation of specific pharmacotherapy outcomes.
  • Perform routine patient chart reviews and patient outreach to accomplish institutional goals on different metrics
Requirements
  • PharmD required, Post graduate year one residency training is required, Post graduate year two residency training in ambulatory care is preferred; OR minimum of 3 years of clinical experience in ambulatory care
  • New York State Pharmacist License required
  • Board Certification in Pharmacotherapy or Ambulatory Care Pharmacotherapy preferred
  • Fluency and proficiency in Spanish language required
