Case Manager

Flushing Hospital Medical Center is a 293-bed, not-for-profit teaching hospital. The hospital offers a variety of services for a culturally diverse community, including Bariatric Surgical Service, Ambulatory Care Center, Women’s Health Center, Wound Care Center, and Chemical Dependence Unit. Our busy on-site full-service pharmacy provides prescription services to discharged, emergency department, and clinic patients as well as employees. We are currently seeking a Case Manager for an immediate full time day shift position.

The Case Manager has the primary duty of providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.  The overall goal of the position is to enhance the quality of patient management and satisfaction to promote continuity of care and cost effectiveness through the integrating of case management, utilization review and management and discharge planning.  Has accountability for the care, coordination and discharge planning of all patients.
