Flushing Hospital Medical Center located in Queens, New York has a full time opening for a Case Manager.

The Case Manager has the primary duty of providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.  The overall goal of the position is to enhance the quality of patient management and satisfaction to promote continuity of care and cost effectiveness through the integrating of case management, utilization review and management and discharge planning.  Has accountability for the care, coordination and discharge planning of all patients.

Requirements

·         NYS RN License. Bachelor’s degree in Nursing, Master’s preferred.

·         2-5 years of clinical experience working as an RN

·         PRI certification preferred or obtained within 3 months of hire

·         BLS Required

·         Knowledge of Allscripts/CarePort Care Management and Milliman Care Guidelines a plus.

·         Basic computer skills, advanced training preferred.

·         Good oral and written communication skills

·         Good analytical and problem solving skills.
