Jamaica Hospital Nursing Home has an exciting career opportunity for an experienced Physical Therapist to perform evaluation, treatment, and resident/caregiver education in a SNF Rehab Department. You will work with a culturally diverse and multi-disciplinary team. Holiday and weekend coverage required.

Job Responsibilities of the Physical Therapist:

  • Administer physical therapy evaluations and treatments under the clinical direction of a licensed physician.
  • Applies age-appropriate evaluation and establishes treatment plans based on subjective and objective findings
  • Applies treatment modalities and techniques within the APTA standards of practice and within established precautions and contraindications 
  • Accurately enters and submits patient-related documentation in a timely fashion 
  • Participates in quality improvement chart reviews 
  • Provides direct supervision to Physical Therapy assistants, Rehab Aid, and Physical Therapy students in accordance set by APTA and the facility.
  • Assisting with developing and implementing discharge planning to ensure safe discharge 
  • Collaborates with interdisciplinary team to help residents achieve goals set forth in the treatment plan
  • Follows departmental policies and procedures. 
Requirements
  • Master of Science degree or DPT degree from an accredited physical therapy program.        
  • NYS Professional License and Registration.
  • EMR competent
  • SNF experience required

SALARY:  $45.47 /hr. - $50.03/hr. DOE
