Responsibilities:
- Collaborate with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs
- Act as an advocate for patients to resolve crises and provide appropriate interventions
- Investigate suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
- Complete a biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
- Refer patients to community or social service programs to meet their complex needs
