Flushing Hospital Medical Center is a 293-bed, not-for-profit teaching hospital. The hospital offers a variety of services for a culturally diverse community, including Bariatric Surgical Service, Ambulatory Care Center, Women’s Health Center, Wound Care Center, and Chemical Dependence Unit. Our busy on-site full-service pharmacy provides prescription services to discharged, emergency department, and clinic patients as well as employees.

Responsibilities:

  • Collaborate with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs
  • Act as an advocate for patients to resolve crises and provide appropriate interventions
  • Investigate suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
  • Complete a biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
  • Refer patients to community or social service programs to meet their complex needs
Requirements

Requirements:

  • Master’s Degree in Social Work required.
  • NYS LMSW licensure required.
  • Ability to observe, identify problems and implement precautionary measures.
  • Independently conduct and complete comprehensives Pycho-Social Assessments to develop interventions.
  • Ability to work well under pressure.
  • Computer literate.
  • Effectively facilitate communication.
  • Skilled in ACS/APS Reporting Knowledge of medical terminology a plus.
