Flushing Hospital Medical Center is a 293-bed, not-for-profit teaching hospital. The hospital offers a variety of services for a culturally diverse community, including Bariatric Surgical Service, Ambulatory Care Center, Women’s Health Center, Wound Care Center, and Chemical Dependence Unit. Our busy on-site full-service pharmacy provides prescription services to discharged, emergency department, and clinic patients as well as employees. We are currently seeking an experienced Clinical Pharmacy Specialist for Internal Medicine for an immediate full-time position.

The Clinical Pharmacy Specialist will round with multi-disciplinary teams for general medicine/surgical patients and provide pharmacotherapeutic management to patients.

·         Practice with other members of the patient care team and focus on the needs of patients.

·         Regularly make rounds as a member of the multidisciplinary patient care team to optimize pharmacotherapeutic managements of patients.

·         Prospectively evaluate and document all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitors the patients’ pharmacotherapeutic regimen for effectiveness and ADEs; and intervenes as needed.

·         Documents clinical activities that include, but not limited to, disease state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities

·         Acts as a liaison between pharmacy, nursing, and the medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines, and pathways.

·         Identify ADEs and assists in their management and prevention, and develops process improvements to reduce drug errors and preventable ADEs.

·         Provide drug information to the patient care team.

·         Participates in training pharmacy students and residents through experiential critical care rotations, where applicable.

·         Contributes to the hospital newsletters and drug monographs on issues related to drug use in Internal Medicine.

·         Implements and maintains departmental policies and procedures related to safe and effective use of drugs in Internal Medicine

·         Provide didactic lectures to health professional residents and students in pharmacology and therapeutics, where applicable.

·         Collaborates with nursing, medical staff, and hospital administration to prepare the Internal Medicine areas for The Joint Commission survey and responds to any deficiencies identified.

·         Provides consultation to hospital committees, such as Pharmacy Therapeutics and Nutrition, when Internal Medicine related issues are discussed.

·         Identifies how drug costs may be minimized through appropriate use of drugs in the Internal Medicine and through implementation of cost-containment measures.

·         Participates in quality assurance programs to enhance pharmaceutical care.

·         Serve as a member of the Pharmacy Management team and participate in management related activities.

Requirements

·         PharmD required, Post graduate year one residency training is required, Post graduate year two residency training in Internal Medicine; or minimum of 3 years of clinical experience in Internal Medicine; or Post graduate year two residency training in Internal Medicine is preferred

·         New York State Pharmacist License required

·         Board Certification in Pharmacotherapy  preferred
