The Care Management Department at Jamaica Hospital Medical Center (JHMC) assists people who need frequent medical attention. We work closely with primary care physicians (PCP), patients, families and health care providers to support care in a patient-centered, community-based way. All of our chronic care management programs strongly support shared decision making with the patient and family, giving patients a voice in their own health care. 

Our Care Management Department has exciting opportunity for a Community Care Assistant to join their team!  Our Community Care Assistant will provide Care Transitions and Care Management services for patients in the community following a recent hospitalization.  Additional responsibilities include: 

  • Provide assessment and formulation of a person-centered care plan and engages with patients and caregivers to meet goals that matter to the patient. 
  • May be required to travel to patients’ homes and community agencies, including medical and/or government offices.
Requirements
  • High School Diploma required, Associate or Bachelor degree preferred. 
  • 2+ years of administrative support experience required.  
  • Strong computer skills and data entry experience. 
  • Excellent organizational and communication skills, both written and verbal required. 
  • Sound research skills, knowledge of medical terminology. 
  • Ability to work both independently and in a team setting needed.
